Summary

Removes "each in a different phase" constraint for Phases 1-4
Multiple issues can now be in triage or planning simultaneously
Adds guidance: process one task per iteration, start new triage only when nothing else can advance

Closes #11

Test plan

Verify concurrency rules allow same-phase issues for Phases 1-4
Verify Phase 5 single-issue limit is preserved
Verify priority order is clear: advance existing > start new

docs: allow parallel triage and planning with priority guidance 79322cf7a7

Remove the "each in a different phase" constraint so multiple issues can
be in triage or planning simultaneously. Add guidance to process one task
at a time per iteration, starting new triage only when no in-flight issue
can be advanced.

Closes #11
