Summary

Broadens Phase 1 trigger to include maintainer-assigned issues, removes undefined "agent's domain"
Adds platform detection rules, permission verification for approvals, and stateless state tracking documentation
Adds timeout escalation for waiting phases and a label state transition table

Closes #1

Test plan

Read updated document end-to-end for internal consistency
Verify all 7 items from issue #1 are addressed
Check label transition table matches phase descriptions

docs: clarify triggers, state tracking, and edge cases 21e77a305f

Address all 7 items from issue #1:
- Broaden Phase 1 trigger to include issues not assigned to another agent
- Remove undefined "agent's domain" qualifier
- Specify origin remote and platform detection rules with fallback
- Add permission verification for approval reactions with API examples
- Explicitly document label-based stateless state tracking
- Add 7/14-day timeouts for Phase 4 and Phase 7 waiting periods
- Add label state transition table

Closes #1
