Summary

Adds step 3 to Phase 7 for detecting, diagnosing, and fixing CI check failures
Adds "Checks Persistently Failing" error handling subsection
Updates Phase 7 gate to explicitly mention CI checks passing

Verify Phase 7 now covers both review comments and check failures
Verify the new error handling subsection follows existing patterns

docs: add handling for failing CI checks in Phase 7 1bda6fda9a

Add step to Phase 7 for monitoring and fixing CI/check failures on merge
requests. Add error handling subsection for persistently failing checks
that follows the existing "Agent Is Stuck" pattern.
